Assessment & Management of the Acute Respiratory Patient in the Community Setting (including issues related to admission avoidance)

Introduction
This module aims to provide healthcare workers with the knowledge and skills to assess, triage and give first line treatment of acutely ill respiratory patients in the community.  It will enable students to develop approaches to assessment, planning and delivery of individualised programmes of care, underpinned by current evidence base
Module Format
This module is run a blended learning basis supported by a comprehensive course pack and four structured study days over 20 weeks, which will include tutorials, online and classroom-based workshops and discussions, ensuring that theory and practice are integrated
Aims
This module will enable healthcare professionals to
  • Analyse and critically evaluate the essential components of respiratory assessment in the acutely ill patient in the community
  • Critically analyse the pathophysiological changes in common respiratory diseases
  • Critically evaluate current multi-disciplinary approaches to organisation and delivery of care and services for patients who are exacerbating but can be cared for in the home
  • Critically evaluate current research and literature relating to the management of acute episodes at home
  • Critically appraise the legal/professional, ethical and moral issues in context of the treatment and care of acute patients in the community setting, that mitigate for or against home management
Course Content
The content of the module includes
  • Applied physiology of the respiratory systems
  • Overview of common respiratory disorders and how the pathophysiological changes that occur impact upon symptomatology
  • Patient assessment, including the principles of respiratory and general assessment, history taking, physical examination
  • Explore the key factors of assessment and application to differential diagnosis
  • The pathway to decline in chronic disease
  • Interpretation of key investigations in the acute patients including, peak flows, pulse oximetry, capillary blood gases and base line observations
  • Triage and immediate treatment including oxygen therapy and emergency medication and monitoring
  • When to admit to hospital
  • Review of local, national and international guidelines that consider the management of patients safely at home
  • Clinical scenarios to consider management issues such as acute asthma, exacerbations of COPD and palliative emergencies
  • Management of exacerbations of COPD, hospital at home and admission avoidance
  • Multi disciplinary teams
  • Exploration of political and financial influences at both national and local level to manage patients at home
  • Ethical, social and legal/professional issues of home management
  • Limitations to practice including accountability
